Breast augmentation is a very popular procedure but breast implants may not be for everyone. Breast Implant removal is an option if you are unhappy with your implants and you wish to restore natural looking breasts.

Why consider Explantation (Breast Implant Removal)?


Most women are very happy with their improved appearance after breast augmentation, but breast augmentation may not be for everyone. There comes a time when some women decide they no longer want their implants. Women may choose to remove (or replace) their implants for any of the following reasons:

  • You are unhappy with the appearance of your breasts
  • You have pain or discomfort
  • Your breasts are too heavy or too big
  • You have capsular contracture (scar tissue) causing hardened breasts
  • You have a rupture or leakage
  • You are interested in adjusting the size of the implant (revision)
  • You simply no longer want the implants

What will my breasts look like after Breast Implant Removal surgery?


All women (and breasts) are different. The majority of women who undergo implant removal are left with a pleasing shape. The breasts will look smaller, but most often they will resemble the appearance before implant surgery. It is important to remember that there are changes in the body that may have occurred since the implants were inserted. Weight changes, pregnancy, nursing and time will all affect the appearance of the natural breast tissue. The need for multiple surgeries and/ or the treatment of complications of implant surgery such as capsular contracture and rupture, may lead to scarring which will also affect the amount of breast tissue and the final appearance.

Will I need a Breast Lift after my Explantation?


Some women are concerned that their breasts will sag, or lose fullness in the upper portion after implant removal…the same concerns that led them to pursue breast augmentation in the first place. Dr. Friedlander is able to address some of those concerns with newer techniques of fat grafting or by lifting the breast tissue through auto-augmentation.  Fat grafting- the transfer of fat from one area of the body where an excess is present, to the breast- can restore fullness to the upper pole of the breast. Auto augmentation- internally lifting the breast tissue – will lift and reshape the breast and restore fullness to the central area.

What can I expect during Breast Implant Removal surgery?


Prior to your surgery, Dr. Friedlander will thoroughly evaluate your needs, and make specific recommendations. She will review the procedure and discuss the various techniques and incision locations.

An explantation procedure is usually performed with an incision placed in the breast (inframammary) fold, most often utilizing the scar from the original surgery. The implant is carefully removed during the procedure. If the scar tissue (capsule) surrounding the implant is thickened or hardened, it may be necessary to remove the entire capsule. This is called a total capsulectomy. If the capsule is very thin and flimsy, some of it may be left behind as removing all of it may damage the surrounding, normal breast tissue. It will adhere to itself as the body heals. Drains are placed in the old implant pocket to help with the healing process. They are removed after a few days.

What is the recovery like after Breast Implant Removal?


The recovery tends to be easier than the original surgery. Dr. Friedlander will provide you with a prescription for pain medication but, since there is no stretching of the muscle or skin from the implant, there is less discomfort and less need for analgesics. You will need to wear a sports bra for several weeks to aid in the recovery. There is no strenuous activity permitted for 4 weeks after the surgery.

It is common to have swelling, and if a lift or fat transfers have been performed, you may find that there are areas of irregularity and firmness in the beginning. As the healing progresses and the tissue settles, these will gradually improve, resulting in a natural shape.
