What are my options for Gynecomastia surgery?

Your gynecomastia surgery may include liposuction, excision, or a combination of both.

  • Liposuction: Often, particularly in men over 50, gynecomastia is associated with fat deposits in the breast. For those patients, liposuction surgery can be performed. Small incisions will be made strategically around the chest, allowing the fatty deposits to be suctioned out through a thin cannula (a hollow tube).
  • Excisional Surgery: Men with excess skin or glandular tissue may require the excision (surgical removal) of the tissue. Incisions are made around the areola or along the natural crease below the breast. This is often the procedure required in adolescents and younger men. This may also be needed in men who have lost a great deal of weight. These incisions may be less extensive depending on the amount of correction required. Through these incisions:
    • The skin is separated from the underlying fat and tissue.
    • Fat cells and glandular tissue are removed.
    • Excess skin is trimmed.
    • The remaining skin is re-draped, and the incisions are closed with sutures.

Your gynecomastia surgery may include liposuction, excision, or a combination of both.

Nipple Reduction for Hypertrophy

Protruding nipples are a common problem that often accompanies enlarged male breasts. To reduce nipple length in men, Dr. Friedlander performs a technique known as nipple reduction. She does this by resecting the excess tissue from the center of the elongated protrusion to shorten its length, then suturing the nipple in place. Because the tissue is darkly pigmented, the scars are practically invisible to the untrained eye.

Gynecomastia surgery is an outpatient procedure, and you will return home to begin your recovery shortly after the surgery is complete. You may develop some bruising and swelling in the chest area following your surgery. This swelling can be minimized with a compression vest that should be worn underneath your clothing for a few weeks. Most men return to work within a few days of the surgery but should avoid strenuous activity or working out for at least four weeks.

  • A wait-and-see philosophy:
    Many young men develop gynecomastia due to the rapid fluctuation in hormones. Occasionally, the excess breast tissue and fat will disappear on their own once the hormones have stabilized.
