General information about reconstructive surgery
Dr. Friedlander may believe you
need reconstructive surgery which may be reimbursable by your insurance carrier.
Insurance carriers vary in what they consider to be reconstructive. Therefore,
when time permits, we submit a letter of predetermination of benefits to the
carrier. The carrier will advise us if they cover the procedure in question. You will be responsible for your
deductible, copayment and any out of pocket expenses and when possible, will be asked
to pay that amount preoperatively. After surgery, we will file an insurance
claim on your behalf and balance bill you for any remaining portion.
If you need a referral to be seen, please obtain this form from your primary
care physician before your appointment.
Is there a charge for the consultation?
Yes, there is. The fee for a consultation for cosmetic surgery is $100, which
is payable at the time of the consultation and deducted from the payment of your surgery fee.

What if I am unsure after my consultation, can I come back?
We want all your questions answered before you decide to have surgery. If you
would like to see Dr. Friedlander again after your initial consultation, you may
do so at no additional cost.

What forms of payment does Dr. Friedlander accept?
Payment is accepted in the form of personal check, money order, cashier's
check, or cash.

How much will the surgery cost?
The cost of surgery
is highly variable, depending on the procedure you select. A fee will be quoted
for the surgery, the anesthesia and the facility after your consultation. This
fee is inclusive and will be reviewed with you. On occasion, additional fees
will be generated, such as the cost of prescription medications, medical
clearance by your personal physician, interpretation and or performance of lab
tests or an unanticipated overnight stay in the facility.

Will my procedure be covered by insurance?
Cosmetic procedures are not covered by insurance. If Dr. Friedlander believes
that your procedure is medically necessary, her staff may send a letter of
medical necessity to your insurance company. Most insurance companies respond in
2-4 weeks about whether or not it is a covered procedure (although this
predetermination process can take up to 6 weeks to complete). If your insurance
plan determines that the procedure is medically necessary, her staff will try to
find out about the level of benefits your plan will pay, as well as your
ultimate financial responsibility to this office.
